COVID-19 Vaccination

San Francisco Department of Public Health COVID-19 Vaccination Site

San Francisco Department of Public Health (SFDPH) is currently making COVID-19 vaccinations available to the 1B tier which includes individuals who work in education. San Francisco State University employees may contact their primary healthcare provider to register for the vaccine. 

SFDPH  – in partnership with Safeway – is also offering the COVID-19 vaccine on campus at San Francisco State University. The County is designating a limited quantity of vaccines at the Mashouf Wellness Center vaccination site for University employees. As new vaccinations appointments are available on campus, Safeway will provide a new, unique link which will be posted on this page (expected either Friday afternoon or early Monday morning, based on availability). Because supplies and appointments are currently limited, employees are encouraged to contact their primary healthcare provider to register for the vaccine, if possible.

**IMPORTANT REMINDERS for all participants**

·       Participants are seen by appointment only. Sign up at the link above, and fill out all forms in advance of your appointment.
·       Fill out the COVID-19 Immunization Consent Form prior to your appointment
·       Bring driver's License/ID, medical and prescription insurance card to the appointment
·       Bring proof of eligibility
·       Wear short-sleeve shirt if possible
·       Do NOT attend clinic if you are feeling unwell or experiencing COVID symptoms
 

Please note that while SF State is hosting the vaccination site, it is not responsible for determining eligibility. Any questions regarding vaccine eligibility should be directed to SFDPH. You can sign up with the City of San Francisco to be notified when you become eligible to receive the vaccine.

Appointments are required, and daily registrations are limited. To receive your vaccination, you must verify your identity, age, residence and/or affiliation with SF State. When you become eligible to recieve your vaccination, please bring your driver’s license, Medicare Part B or other prescription and medical insurance card if you have them, and your SF State ID with you to your appointment. Please be aware that the protocols around vaccination distribution continue to evolve, as does the availability of vaccine supply. While the University is hosting this vaccination site, it does not play any role in the distribution or availability of the vaccine.

Free Transportation

San Francisco Municipal Transportation Agency (SFMTA) is offering free, roundtrip transportation for people traveling to receive their COVID-19 vaccine.
 
Anyone traveling to receive their vaccination can ride Muni to and from their appointment for free, regardless of which site they go to. This includes the City’s high-volume vaccination sites, neighborhood vaccine access sites, Department of Public Health clinics, pharmacies offering vaccinations, and vaccination sites outside of San Francisco.
 
Customers on Muni traveling to and from a COVID-19 vaccination site will need to show their appointment confirmation or vaccine card as proof of payment. Electronic appointment confirmations, such as an email or photo will be accepted.
